I’m trying to determine the P4P improvement rate for 4 physicians based on relative, benchmark, and improvement performance. If physician #1 had 60% of their patients with at least 2 HbA1c tests in the past year, how would I calculate their P4P bonus ($5000) based on the first half of the year experiencing 60% and the second half 76%?
